Overview
The Community Health & Wellness Program is ASCHWC’s flagship initiative — a movement designed to bridge the gap between home-based care and community-wide wellbeing. We believe true wellness begins at home but grows strongest when shared across families and neighborhoods.
Through preventive health education, early-detection screenings, and compassionate outreach, ASCHWC helps individuals and families take control of their health — body, mind, and spirit. Each home visit, workshop, and partnership brings us closer to our vision of healthier, happier, and more resilient communities across Pennsylvania.
Why It Matters
Many preventable illnesses persist simply because people lack access to basic information or routine screening. This program tackles those barriers head-on by bringing wellness directly to the community — into homes, schools, workplaces, and senior centers.
Healthcare should not start in hospitals; it should start with awareness, good habits, and preventive action. By emphasizing education and regular health monitoring, the Community Health & Wellness Program reduces disease risks and encourages a culture of shared responsibility for wellbeing.
